Ultrasound checkout
Lubbock and Odessa Ultrasound Equipment Checkout
To check out ultrasound equipment, or to request practice of ultrasound at the Lubbock
or Odessa SimLife Center, or the Lubbock and Odessa Libraries, please fill out the
following form. All equipment is available for 24 hour checkout and is to be used
for educational purposes only. EQUIPMENT IS NOT TO BE USED FOR DIAGNOSTICS. Patron is responsible for the equipment that is checked out.
IMPORTANT:
- The libraries have been instructed to not check an ultrasound out until we have received the official approval notice from the SimLife Center. Approvals can take up to 72 hours.
- SimLife approval is based upon prior documented knowledge and skills of ultrasound technology.
- Previous SimLife approvals do not authorize future checkouts. Every individual checkout requires a new SimLife approval.
